As of V32, you can insert a QR code directly into your schematics and PCBs. In the "Actions" menu there is the item "Insert QR code...". In the following dialog you can enter the text, the size and the layer for the code. After [OK] the code hangs square on your mouse and you can place it with M1:

The QR code is placed internally as an image on a rectangle (square). If you click on the [Version] button, <version> will be inserted in the text. This is the current version of your project and the QR code will be refreshed each time you add it. Since the image of the QR code is prepared on our server, you must be online with TARGET at that moment. 

Depending on the color of the output on the board, it may be necessary to output the QR code inverted. Some QR code readers (cell phones) also invert the image themselves, depending on the contrast, but not all can.
